Staff - Report of Workplace Violence
In order to ensure that we maintain accurate records of incidents of workplace violence, as outlined in our Workplace Violence Prevention Plan, please complete a Report of Workplace Violence any time you experience violence in the work place. Please note that this form goes to the Director of Student Services and is for the purpose of recording incidents and making adjustments to prevent future incidents from occurring; you should still always contact your supervisor/site admin immediately if you experience violence in the workplace.
